I purchased a $100M policy through NewYork Life. As of three years ago besides a physical that they performed they did not require any additional data as to a PPL and that was noted on the application. The premimum was based soley on my age and health. That was the expensive part!
I undersatnd that if the policy was higher then it may have been in question.
